To be fair, his performances at the end of last season, against Napoli, Benfica, Barcelona and Liverpool were fantastic. Not consistent enough to be in that team though.

Ramires is there because he was incredible in the Champions League - was brilliant in stopping Maggio against Napoli and setting up Drogba's goal and having a hand in Ivanovic's winner. He had a huge hand in Kalou's winner at Benfica as well as stopping Nico Gaitan, and his two displays against Barcelona were top class - the assist for Drogba, and the amazing goal above.
