Tackling with both feet is still common place in the game. We get to see it about once every other game, maybe more. Even though it's a sending off offensive, players are for some reason willing to take the risk.
From a personal perspective, I can't understand it. It's just a completely unnatural way to tackle. I played at right back for many years and never even contemplated going in with both feet (might have had something to do with my aversion of landing heavily on my bum )
So my questions are:
1. Why do players do it? Surely it can't always be to injure the opposing player, else we have a large number of psycho out there playing the game.
2. Regardless of what level you play or have played at, have you ever gone into a tackle 2 footed? Why did you do it? To win the ball or take out the other guy with intent to cause bodily harm or both?
3. Have you ever been a recepient of a 2-footed tackle? How did it feel? Did you think the player was genuinely after the ball or after you?
PS Received one of these once and the guy defo had it out for me... I got a free kick for my troubles
The two-footed tackle

I don't remember ever actually going in 2 footed, but the only explanation for it being premeditated, would be that they are fully aware of the damage they will cause by doing it, otherwise there would be no reason.

I think it comes from looking to make the tackle with one foot but then as you do the motion havin the ball move and requiring the other foot to get involved
A bit like when you try to kick a ball that ends up being head height when it reaches you, resulting in a sort of sideways bicycle kick and you wondering where that agility suddenly came from

For some reason, the second part of that, gave me an image of a slightly overweight middle aged man trying to do some sort of sideways bicycle kick at the park with his kids while saying 'I've still got it'
You probably aren't any of those things, but you gave me a laugh this slow afternoon

It's basically a coward's tackle that is done so as not to lose face because the player knows he has been beaten to the ball.
It's either done because a player is desperately trying to make up for a mistake and is genuinely trying to get ball or it is done deliberately with intent to injure or protect themselves.
Imagine you are running after something and as you get closer you realise you are not going to make it. It's a basic human instinct to jump for it, whatever it is.

I've done it games before.. some referees will let you get away with it, some will get the cards out even if you win the ball.
For me it is usually towards the end of a game, because as I'm tiring, jumping 2 foot seems like a quicker option than sliding 2 feet. In addition to pressure from the way the game has been going and also the fact that I'm not a defender.
Well those are my excuses anyway.
