Chelsea had a pretty bad title collapse under Ancelotti iirc.

2010/11 we stormed through autumn then finished second behind United. Harsh sacking
It's probably a very varied question that one. Someone might be 4 points clear in September then finish in the bottom 10.
The old biggest lead lost one is easier to do. I know we (Arsenal) lost a big lead to United during the early Wenger years, maybe even the season they won the treble?
Newcastle also had that famous bottle job under Keegan. No doubt there's plenty more!

Were comfortably in 1st place until we lost key players to injuries and then went through a couple of months where we could barely win a game and went into freefall. We turned things around that season eventually though, dunno whether Liverpool will do the same.

Liverpool finished 18 points ahead of City last season.
City are currently 19 points ahead of Liverpool.

Title collapse is different from title defence
How so? You either have the title and collapse or you don’t.
Also as much as it pains me to say it but Atleast Liverpool have excuses
Title defence is referring to winning the title one year and then how close you come to winning it next year
Title collapse is about being in pole position to win the title in a given year and failing to win it from that position.
