it feels like I need an axe straight through the top of the shoulder, which is something I haven't thought of for a good fee years. not pleasant.

I had a shoulder muscle in spasm for about 7 years after I finished university. Constant pain and in a knot. Used to get my wife to use her elbow to massage it to try and relieve some of the pain. Never really worked. Got some heavy ass weights, and done curl and press for a few months, it has never bothered me after that. Despite the fact the weights are gathering dust.
it's not a muscle spasm, it's a trapped nerve. either of the solutions you mention would be an absolute disaster for my problem. glad your problem has cleared up.
acupuncture would also probably play merry hell with it, but it's the sort of thing that might be worth trying once if it lasts a long time again.
